Use case

PCP referrals

Lumeon enables PCPs to refer electronically from within their EHR, then track progress via our portal. The platform can send follow-up forms to referrers (or patients) to capture missing information and chase according to timers in the pathway. Meanwhile, the recipient care team is guided through a path of critical activities and presented with a real-time view which tasks have been completed and what is outstanding (from financial checks to onsite screening).  Once the referral is approved, the PCP is emailed and the patient is assigned to a risk-stratified pathway.


Referrals into outsourced diagnostic providers

CPM enables the diagnostic provider to establish triaging protocols that screen the referral for criteria e.g. patient age, previous metal in their leg, allergy to contrast etc. which could make it unsuitable. Eliminating the need for highly paid medical staff to review each and every case, the platform enables lower-tier administrators to run through checklists with patients in order to either rapidly book them in for procedures. If any potential risk factors are detected, it will auto-escalate the referral to more senior medical staff for review.
